The Australian Cancer Survivorship Centre (ACSC) invites registrations for this virtual Victorian Cancer Survivorship Community of Practice.
The forum brings together health professionals with an interest in cancer survivorship and the implementation of new models of care and provides an opportunity to discuss key aspects of cancer survivorship.
The forum will include:
- Welcome – Helana Kelly, Manager, ACSC
- Victorian Cancer Plan 2024-2028 – Lauren Cook-Zabrdac, Manager Policy Projects and Engagement, Hospitals and Health Services, Department of Health
- Consumer Advocate – Michael Powell
- PHN Shared Care Toolkit – Catherine Lambert, Statewide Coordinator, Cancer Shared Care, Gippsland Primary Health Network
- Cancer Survivorship Nursing Capabilities – Gemma McErlean, Associate Professor in Nursing, University of Wollongong
- Evaluation and close
