For patients to be seen by the Neuroendocrine tumour (NET) Service, a comprehensive referral is required, including:
- A completed Peter Mac referral form – see referral form and further information
- Comprehensive background information and reason for referral, including details of history and current management
- All relevant imaging reports, including the date and provider
- All relevant operation and Endoscopy reports
- All relevant histology reports, including the date and provider
- Lab results including relevant tumour markers, or hormone levels (for example, Chromogranin A, 5HIAA, BNP and so on)
- Echocardiogram reports (if done)
All referrals should include a completed referral form, and be emailed to
