Your Patient Navigator will be an important member of your care team. We will pair you with a Patient Navigator when you join us as a patient. They will help ensure that you receive the best care. They will help you liaise with your team. Your team can include doctors, nurses, pharmacists, and allied health practitioners. For example, they can help you book or cancel appointments with many of our services.
When your Patient Navigator can help you
You can ask them to help you book or cancel an appointment. Such an appointment can be at one of the following services:
- Specialist Clinics
- Allied Health - including
- Physiotherapy
- Social work
- Occupational therapy
- Exercise physiology
- Speech therapy
- Nutrition and dietetics
- Psychology
How to use your Patient Navigator
Patient Navigators are available 9am to 4:30pm Monday to Friday.
Your specific Patient Navigator depends on your diagnoses or treatment group. Find out who they are and contact them by following these instructions.
- Open the Patient Navigator Contact list or the Haematology Patient Navigator Contact List
- Look for your right category in the first column
- Contact the Patient Navigator in the column on the right by:
- Phone - you can leave a message on an answering machine if the Patient Navigator doesn’t answer the phone
- They will return your email, or any voice mail, as soon as they are able
- Tell them what service you would like to book or cancel
When not to use your Patient Navigator
You will not be able to use a Patient Navigator:
- In the evenings
- On weekends
- To book or cancel an appointment for some services. Instead, you will need to call them yourself. These services include:
