As a young person receiving care at the Royal Children’s Hospital (RCH), there comes a time when it becomes necessary to transition to an adult health service.
The AYA-PACT is a pilot program designed to assist young people who have completed treatment for childhood cancer at RCH in preparing for this transition to adult care at Peter Mac. This program includes a multidisciplinary transition clinic, where guidance is provided to both the young person and their family on what to expect within the adult healthcare system. It also helps in accessing the right information and support, and prepares them for meeting their new doctors.
Through this comprehensive support, young people can feel more confident and comfortable as they navigate their move to adult health services.
